EXPRESS CONSENT FORM
Prof. Dr. Hasan Göçer Clinic (“Clinic”), acting as the data controller, processes the personal data of data subjects in accordance with the principle of protecting the privacy of private life, among others; in accordance with the Law on the Protection of Personal Data No. 6698 ("Law"), in accordance with the Law and within the framework of the processing conditions listed in the Law, as detailed in the privacy notice available on the https://hasangocer.com.tr website; and will be transferred to third parties to the extent permitted by law.
IDENTITY OF THE DATA CONTROLLER AND ITS REPRESENTATIVE
In accordance with the Law, your personal data will be collected and processed by Prof. Dr. Hasan Göçer, acting as the data controller, within the scope described below. PURPOSES OF PROCESSING PERSONAL DATA
Your personal data will be processed in accordance with the conditions set forth under the heading “Conditions for the Processing of Personal Data” in Article 5 of the Law, and to the extent permitted by your explicit consent, for the purposes of protecting public health, providing preventive medicine, medical diagnosis, treatment, and care services, to manage appointment processes through the call center, to store your personal data in physical and electronic archives maintained by domestic and international service providers, to conduct access-related activities, to carry out financial and accounting activities, evaluating requests and complaints, and carrying out all types of legal and financial advisory services—we may record, store, update, transfer to third parties, classify, and process your personal data within the scope of these activities.
TRANSFER OF PERSONAL DATA
Personal data belonging to data subjects may be transferred to: the Ministry of Health, its subordinate units, private insurance companies, the Social Security Institution, the General Directorate of Security and other law enforcement agencies, the General Directorate of Population, the Turkish Pharmacists’ Association, judicial authorities, domestic and international laboratories with which we collaborate for medical diagnosis and treatment, medical centers, other clinics and healthcare institutions, authorized legal representatives, consulting attorneys, tax advisors, and auditors, regulatory and supervisory bodies and official authorities, and independent accountants and financial advisors in the context of billing procedures.
SHARING OF PERSONAL DATA WITH THIRD PARTIES LOCATED ABROAD
Your personal data may be shared abroad within the scope of utilizing storage and archiving services, specifically with information technology companies providing infrastructure services abroad and companies providing archiving services, as well as in the context of email addresses used to contact relevant parties being hosted in data centers located abroad that form part of the email service infrastructure.
METHOD OF COLLECTION AND LEGAL BASIS FOR THE PROCESSING OF PERSONAL DATA
Your personal data is collected by us electronically, following the obtaining of your explicit consent, for the purpose of ensuring that the services provided by our Clinic are performed completely and accurately in accordance with the law. Your personal data is processed based on the legal ground of “explicit consent” as stipulated in Article 5(1) of the Law.
YOUR RIGHTS UNDER THE LAW
As data subjects, you may submit requests regarding your rights to our Clinic by filling out the relevant form. In this context, data subjects have the following rights:
- Find out whether personal data is being processed,
- Request information regarding the processing of their personal data, if it has been processed,
- To learn the purpose of the processing of personal data and whether it is being used in accordance with that purpose,
- To know the third parties to whom personal data has been transferred, whether within the country or abroad,
- Request the correction of personal data if it has been processed incompletely or incorrectly,
- To request the erasure or destruction of personal data if the grounds for processing no longer exist, even if the data was processed in accordance with the law and other relevant legal provisions,
- To object to a decision made solely through the automated analysis of processed data that results in a negative outcome for the individual,
- To request compensation for damages suffered as a result of the unlawful processing of personal data.
Prof. Dr. Hasan Göçer guarantees that, in the event of any changes to the applicable legislation and any inconsistency between this text and such changes, the provisions of the legislation shall prevail.
